Physical Exams
Guidelines
Edina Schools Health Services encourages periodic health exams since prevention and treatment of health conditions will enhance student's attendance and performance in school. Physical examinations are recommended at kindergarten entry. They are not required. Please consult with your health care provider regarding specific recommendations for preventive care visits.
Physical Exam/Health History Forms are distributed to kindergarten students in the kindergarten registration packets. Physical Exam Forms are also distributed to sixth and ninth grade students, in the spring of the year. They can also be obtained through the health office or online at any time.
Athletic Physical Exams
Students participating in school athletics must follow the athletic registration process. Athletic registration packets include forms for registration, High School League rules, and specific registration information. An updated Physical Exam Form is required every three years and must be submitted on the High School League Physical Examination Form. A copy of this form is also included in the registration packet. Registration packets can be obtained in the Athletic Office or downloaded from the Edina High School Athletic site.
